What is Short Bowel Syndrome (Surgical Approach)?

Short bowel syndrome (SBS) is a malabsorption syndrome arising when the residual small intestine is too short to provide adequate absorption. It typically develops when less than 200 cm of small bowel remains.

The most common causes are mesenteric ischemia, Crohn's disease, trauma, volvulus, and radiation enteritis. The presence of the colon, preservation of the ileocecal valve, and type of remaining bowel significantly influence prognosis.

Treatment is staged through adaptation phase, nutritional support, medical therapy (GLP-2 analogues), and if needed surgical rehabilitation or intestinal transplantation.

Symptoms

Chronic diarrhea and steatorrhea
Weight loss and malnutrition
Dehydration and electrolyte imbalance
Vitamin and mineral deficiencies
Weakness and fatigue
Osteoporosis
Increased risk of gallstone and kidney stone formation

Risk Factors

History of mesenteric ischemia
Extensive bowel resection
Crohn's disease
Radiation enteritis
Traumatic bowel loss
Congenital anomalies
Repeated surgeries
