What is Scrotal Hernia?

Scrotal hernia is the extension of a long-neglected, large indirect inguinal hernia from the inguinal canal into the scrotum. Bowel loops, omentum, and even the bladder may be located in the scrotum. Because it is mostly painless, patients may delay surgical consultation.

In giant scrotal hernias, the bowel segment remains in the scrotum for a long time, leading to adhesion development, and testicular vascular structures may be at risk. Preoperative evaluation with CT and ultrasound determines content, vascular structure, and bowel status.

Surgical repair is performed with open Lichtenstein or Bassini techniques; laparoscopic repair is technically difficult in large scrotal hernias.

Symptoms

Large, soft, painless swelling growing in size in the scrotum over time
Swelling that does not completely disappear when lying down
Difficulty walking and sitting (in giant hernias)
Thinning and ulceration of scrotal skin (in very large hernias)
Sudden pain and tenderness in strangulation

Risk Factors

Long-term neglected groin hernia
Elderly men
Chronic constipation and increased intra-abdominal pressure
Difficulty accessing surgery due to socioeconomic barriers
Connective tissue weakness

When to See a Doctor?

  • Progressively growing swelling in the scrotum
  • Strangulation signs: sudden pain, hardness (emergency)
  • Walking difficulty and limitation of daily activity
  • Thinning or color change of scrotal skin

Preoperative CT for content and vascular structure assessment

Open inguinal hernioplasty: Lichtenstein mesh or Bassini technique

Bowel reduction: careful dissection if adhesion is present

Orchiopexy or oophorectomy: if testicular/ovarian vascularity is at risk

Preoperative pneumoperitoneum to increase abdominal capacity in giant hernias

Postoperative testicular blood flow should be assessed by ultrasound
