What is Roux-en-Y Gastric Bypass?

Roux-en-Y gastric bypass creates a small (~30 mL) gastric pouch anastomosed to a Roux limb of jejunum, bypassing the distal stomach, duodenum, and proximal jejunum.

Mechanism combines restriction (small pouch), malabsorption (bypassed proximal small bowel), and significant neurohormonal changes (GLP-1, PYY, ghrelin, bile acids, microbiome).

Particularly effective for type 2 diabetes remission (60–80% at 2 years) and severe GERD, making it preferred over sleeve gastrectomy in these contexts per ASMBS/IFSO 2022.

Typical excess weight loss is 60–80% at 2 years with durable improvement in diabetes, hypertension, dyslipidemia, obstructive sleep apnea, and quality of life.

Symptoms

Indications for gastric bypass:
BMI ≥40 kg/m² or BMI ≥35 with comorbidities
Severe GERD with obesity (bypass preferred over sleeve)
Type 2 diabetes with obesity — highly effective for remission
BMI ≥30 kg/m² with uncontrolled metabolic disease
Failure of conservative weight management

Risk Factors

Anastomotic leak (1–2%) at gastrojejunostomy or jejunojejunostomy
Internal hernia (2–5% lifetime risk — closure of mesenteric defects essential)
Dumping syndrome (early and late) with high-carbohydrate meals
Nutritional deficiencies — B12, iron, calcium, vitamin D, thiamine, folate, protein
Marginal ulcer at gastrojejunal anastomosis
Cholelithiasis from rapid weight loss
