What is Robotic Bilateral Inguinal Hernia Repair (TAPP)?

Robotic TAPP repair is a minimally invasive transabdominal approach to inguinal hernia where pneumoperitoneum is established, the peritoneum is incised above the hernia defect, the preperitoneal space is dissected to expose the myopectineal orifice of Fruchaud, and a polypropylene mesh is placed to cover indirect, direct, and femoral spaces, then the peritoneal flap is sutured closed. The robotic platform offers articulating instruments and 3D vision favorable for bilateral and recurrent repairs.

Bilateral inguinal hernias and recurrent hernias are the strongest indications for laparoscopic or robotic TAPP because both sides can be addressed through the same trocar setup, and the preperitoneal mesh covers the entire myopectineal orifice including occult contralateral defects. Robotic suturing facilitates secure peritoneal closure, reducing the risk of bowel adhesion to mesh.

Outcomes match open Lichtenstein for recurrence and chronic pain at 1 year and exceed open repair for early postoperative pain, return to work, and cosmesis. Disadvantages are higher cost and need for general anesthesia. Mesh fixation with absorbable tacks, glue, or self-fixating mesh reduces postoperative pain compared to permanent fixation.
