Right Hepatectomy

Major hepatobiliary surgical procedure in which the entire right lobe of the liver (segments V-VIII) is removed.

What is Right Hepatectomy?

Right hepatectomy is a major hepatic resection in which segments V, VI, VII, and VIII (Couinaud classification) constituting the right lobe of the liver are removed en bloc. It involves approximately 60-65% of the liver volume and is performed in experienced hepatobiliary centers.

The procedure includes isolation of the right hepatic pedicle (right hepatic artery, right portal vein, right hepatic duct), control of the right hepatic vein, and parenchymal transection. Pringle maneuver, anatomical, or intermittent occlusion is used to reduce intraoperative bleeding.

Indications include hepatocellular carcinoma, colorectal metastasis, intrahepatic cholangiocarcinoma, hepatic adenoma, focal nodular hyperplasia, and hydatid cyst — benign or malignant large masses. Adequate liver reserve (future liver remnant >30%) and good functional status are required.

Risk Factors

Cirrhotic liver (Child-Pugh A preferred)
Right portal vein thrombosis
Tumor invasion of the right hepatic vein
Inadequate future liver remnant (<30%)
Hepatic dysfunction
Coagulopathy and thrombocytopenia
Advanced age and comorbidities
