What is Retroperitoneal Sarcoma Surgery?

Retroperitoneal sarcomas are rare malignant tumors that originate from the retroperitoneal soft tissues. The most common subtypes are well-differentiated and dedifferentiated liposarcoma, leiomyosarcoma, and solitary fibrous tumor. Most are diagnosed late and may be at large dimensions.

Surgical management includes complete (R0) resection of the tumor en bloc with multivisceral resection if necessary (kidney, colon, pancreas, spleen, vessel, muscle). Recurrence rates are 50-60%, and treatment in experienced centers offers a survival advantage.

Preoperative planning includes detailed imaging (MRI/CT), percutaneous biopsy, multidisciplinary tumor board evaluation, neoadjuvant radiotherapy (selected cases) and surgery. Functional reconstruction is required for vascular and muscular invasion.

Symptoms

Painless abdominal mass
Abdominal distension and discomfort
Early satiety and weight loss
Lower-extremity edema and pain
Renal compression and hydronephrosis
Symptoms of vascular compression

Risk Factors

Li-Fraumeni syndrome
Neurofibromatosis (NF1)
Previous radiation exposure
Familial adenomatous polyposis (FAP)
Chronic lymphedema (Stewart-Treves)
Genetic predisposition
