What is Ureteral Obstruction Due to Retroperitoneal Fibrosis?

Retroperitoneal fibrosis (RPF) most often forms a fibrous tissue mass in the aortoiliac region, pulling the ureters medially and causing obstruction. This is an important complication followed and treated by urology.

Ureteral involvement may be unilateral or bilateral; it presents with hydronephrosis, flank pain, and deterioration of renal function. It develops as idiopathic (Ormond's disease) or secondary forms.

Urgent management focuses on ureteral drainage; subsequent treatment of the underlying disease (steroids, immunosuppressants, surgical ureterolysis) is then planned.

Symptoms

Flank or back pain
Weakness and weight loss
Nausea and loss of appetite
Decreased urine output
Findings of hydronephrosis
Leg edema
Hypertension

IgG4-related disease
Ergot derivatives and certain medications
Smoking
Radiotherapy
Autoimmune diseases
Malignancy
