What is Retinal Detachment?

Rhegmatogenous: retinal break allows vitreous fluid to accumulate beneath the retina.

Tractional: fibrovascular membranes (as in proliferative diabetic retinopathy) pull the retina off.

Exudative: fluid accumulates from choroidal/retinal disease without a break (tumor, inflammation).

Peripheral detachment may progress centrally; macular involvement critically affects visual prognosis.

Symptoms

Sudden onset of flashes (photopsias)
New or increasing floaters
Dark curtain or shadow advancing across the visual field
Blurred or distorted vision
Central vision loss when macula detaches
Absence of pain is typical

Risk Factors

High myopia (thin peripheral retina, lattice degeneration)
Prior cataract or intraocular surgery (pseudophakia)
Ocular trauma
Family or personal history of retinal detachment
Posterior vitreous detachment (PVD) in middle age
Lattice degeneration and peripheral retinal tears

  • New symptoms in fellow eye after prior detachment

Treatment Methods

Retinal breaks without detachment: laser retinopexy or cryotherapy

Pneumatic retinopexy: intraocular gas bubble and posturing for selected detachments

Scleral buckling: indentation of the globe to close breaks

Pars plana vitrectomy with gas or silicone oil tamponade (often combined with buckle)

Proliferative vitreoretinopathy may need complex vitrectomy with membrane peeling
