What is Renal Cell Carcinoma with Inferior Vena Cava Thrombus?

Locally advanced renal cell carcinoma can extend through the renal vein into the inferior vena cava (IVC), and in advanced cases reach the right atrium. This condition occurs in 4-10% of all RCC cases.

Mayo classification defines thrombus level: Level I (renal vein), Level II (infrahepatic IVC), Level III (intrahepatic IVC), Level IV (suprahepatic IVC/right atrium). Surgical complexity and required team (cardiothoracic + liver + urology) increase with level.

Symptoms

Flank pain
Haematuria
Lower extremity oedema (IVC obstruction)
Caput medusae (collateral veins)
Constitutional symptoms (weight loss, fatigue)
Pulmonary embolism (rare)

Risk Factors

Locally advanced RCC (T3-T4 stage)
Tumour size >7 cm
Right kidney location (more frequent)
Sarcomatoid differentiation
von Hippel-Lindau syndrome
Smoking and obesity
