What is Red Blood Cell Morphology Abnormalities?

Red blood cell morphology abnormalities are shape size or color variations of erythrocytes seen on peripheral blood smear that provide important diagnostic clues. Systematic smear review remains essential despite advances in automated counters for evaluating anemia and hematologic disease.

Common shape abnormalities include spherocytes in hereditary spherocytosis or autoimmune hemolysis sickle cells in sickle cell disease target cells in liver disease and thalassemia schistocytes in microangiopathic hemolytic anemia and acanthocytes in liver disease or abetalipoproteinemia. Each pattern suggests specific differential considerations.

Size abnormalities include microcytes suggesting iron deficiency or thalassemia and macrocytes suggesting B12 folate deficiency or myelodysplasia. Inclusions like Howell Jolly bodies indicate splenectomy or hyposplenism while basophilic stippling suggests lead poisoning or thalassemia. Smear correlation with clinical presentation guides further workup.
