What is Radical Cystectomy?

Radical cystectomy is the standard treatment for muscle-invasive bladder cancer. In men, the bladder, prostate, and seminal vesicles are removed; in women, the bladder, uterus, ovaries, and the anterior wall of the vagina are removed. Pelvic lymph node dissection accompanies the procedure.

Urinary diversion is required after the bladder is removed. The main options are an ileal conduit (most common), an orthotopic neobladder (artificial bladder), and a continent catheterizable pouch. Each method has its own advantages and disadvantages.

The operation can be performed open, laparoscopically, or robotically. Modern enhanced recovery after surgery (ERAS) protocols have reduced length of hospital stay and complications. Long-term follow-up evaluates oncologic and functional outcomes.
