What is Pilonidal Sinus Treatment?

Pilonidal sinus is a chronic infectious tract in the sacrococcygeal region usually triggered by trapped hair follicles and skin debris.

It is most common in young men with hirsute, sedentary or sweaty lifestyles.

Treatment ranges from simple lay-open surgery to flap reconstruction (Limberg, Karydakis) and minimally invasive techniques such as laser ablation (SiLaT/SiLaC) and endoscopic pilonidal sinus treatment (EPSiT).

Symptoms

Painful intergluteal swelling
Pus or blood discharge from skin pits
Recurrent abscess formation requiring drainage
Foul-smelling persistent drainage
Sitting discomfort and difficulty bending
Local cellulitis and rarely systemic infection

Risk Factors

Young adult males (15 to 30 years, peak)
Excessive hair growth and obesity
Sedentary occupation, prolonged sitting
Poor local hygiene and friction injury
Family history of pilonidal disease

When to See a Doctor?

If you experience any of the following symptoms, seek medical attention promptly:

  • Recurrent intergluteal swelling and discharge
  • Acute abscess formation requiring drainage
  • Persistent draining sinus despite antibiotics and local care
  • Disabling pain affecting work or daily activity

Treatment Methods

01
Acute abscess: incision and drainage with antibiotic cover
02
Chronic sinus: definitive surgical excision
03
Off-midline flap repairs (Limberg, Karydakis) reduce recurrence and improve healing
04
Minimally invasive options: SiLaT/SiLaC laser ablation, EPSiT endoscopic treatment
05
Postoperative meticulous hygiene, hair removal (laser depilation, shaving) to reduce recurrence
06
Avoid prolonged sitting, weight reduction and dietary improvement for chronic prevention
