What is Pheochromocytoma Surgery?

Pheochromocytoma is a tumor, mostly benign, originating from chromaffin cells of the adrenal medulla and secreting catecholamines (epinephrine, norepinephrine). Extra-adrenal forms are called paragangliomas.

It shows a high rate of genetic transmission (MEN2, VHL, NF1, SDH mutations). Therefore, genetic evaluation is recommended after diagnosis.

Surgery is the only curative treatment; operating without appropriate preoperative medical preparation (alpha blocker, fluid loading, and beta blocker if needed) increases the risk of intraoperative hypertensive crisis.

Symptoms

Paroxysmal hypertension
Headache
Palpitations and tachycardia
Sweating attacks
Anxiety and tremor
Weight loss
Orthostatic hypotension

Risk Factors

MEN2, VHL, NF1 syndromes
SDHB/SDHD mutations
Family history
Previously identified paraganglioma
Detection of incidentaloma
Resistant hypertension at a young age
Recurrent hypertensive crises

Preoperative alpha blocker (phenoxybenzamine/doxazosin) for 10-14 days

Fluid loading and addition of a beta blocker if needed

Laparoscopic adrenalectomy (preferred method)

Open surgery for large, invasive, or extra-adrenal tumors

Intraoperative blood pressure control

Postoperative monitoring for hypoglycemia and hypotension
