What is Perianal Abscess?

Perianal abscess is a purulent collection that forms when the cryptoglandular structures in the anorectal region become obstructed and infected. It most often occurs at the perianal, ischiorectal, intersphincteric, or supralevator locations.

It initially presents with pain, swelling, redness, and fever in the perianal area. Pain is exacerbated by sitting and defecation. In abscesses with deep localization, external findings may be absent and diagnosis may be delayed.

Treatment is urgent surgical drainage. Antibiotic therapy alone is insufficient and evacuation of the abscess is mandatory. Approximately half of patients later develop an anal fistula and require further treatment.

Symptoms

Severe pain in the perianal area
Swelling and redness
Fever and chills
Difficulty sitting
Increased pain with defecation
Purulent discharge
Malaise and fatigue

Immunosuppression
HIV infection
Anal fissure
Male sex
Steroid use
