What is Penile Fracture?

Penile fracture is the rupture of the tunica albuginea during trauma to the erect penis. It is most commonly encountered during sexual intercourse, particularly in awkward positions.

The tear most often occurs in one of the corpora cavernosa; rarely the urethra is also affected. The history is typical: a snapping sound during erection, sudden pain, rapid loss of erection, and penile swelling (eggplant deformity).

Early surgical repair is the standard treatment; in delayed cases, the risk of erectile dysfunction, penile curvature, and plaque formation increases.

Symptoms

Sudden snapping sound during intercourse
Sudden severe penile pain
Rapid loss of erection
Rapidly developing penile swelling and ecchymosis
Eggplant appearance
Hematuria or urethral bleeding (in urethral injury)
Inability to urinate

Risk Factors

Vigorous sexual activity
Awkward intercourse positions
Direct trauma to the penis
Forceful bending during masturbation
Limited sexual experience
Previous penile trauma
Peyronie's disease
