What is Pelvic Lymphadenectomy (Bladder/Prostate Cancer)?

Pelvic lymphadenectomy (PLND) is the surgical removal of pelvic lymph nodes in bladder or prostate cancer. It is performed both for staging and for therapeutic clearance of micrometastases.

Extended pelvic lymphadenectomy (ePLND) covers the external iliac, obturator, internal iliac, presacral and common iliac nodes, while standard PLND addresses only the obturator and external iliac stations.

ePLND is part of the standard treatment in muscle-invasive bladder cancer with proven survival benefit. In intermediate- and high-risk prostate cancer extended PLND is recommended. Robotic surgery has simplified PLND.

Symptoms

Muscle-invasive bladder cancer (cystectomy plus PLND)
Intermediate- to high-risk prostate cancer (radical prostatectomy plus PLND)
Suspicious pelvic lymph node on CT or MRI
Bladder cancer staging
Prostate cancer staging
Elevated PSA or Gleason score

Risk Factors

Lymphocele formation
Deep vein thrombosis
Lower-extremity lymphoedema
Obturator nerve injury
Vascular injury to the iliac vessels
Pelvic lymph leak
