What is Paroxysmal Cold Hemoglobinuria?

An autoimmune hemolytic anemia mediated by IgG biphasic antibodies that bind erythrocytes in the cold and fix complement upon warming

Antibodies target the P antigen on red blood cells and trigger massive intravascular hemolysis

Most cases occur in children following viral upper respiratory or gastrointestinal infections

Adult forms may follow syphilis or be idiopathic in lymphoproliferative disorders

Diagnosis relies on positive Donath-Landsteiner test demonstrating biphasic hemolysis in vitro

Symptoms

Sudden onset of fatigue, pallor, and jaundice after cold exposure
Dark or red-brown urine reflecting hemoglobinuria within hours of exposure
Abdominal pain, back pain, fever, and chills accompanying acute episodes
Headache, nausea, vomiting, and anemia in severe attacks
Resolution within days as antibody titers decline after infection clearance

Risk Factors

Recent viral infection including measles, mumps, varicella, mycoplasma, and influenza
Pediatric age group with peak incidence between 2 and 5 years
History of syphilis in adult cases (now uncommon with treatment)
Cold ambient temperatures, swimming, or other cold exposures
Lymphoproliferative disorders or non-Hodgkin lymphoma in adults

When to See a Doctor?

If you experience any of the following symptoms, seek medical attention promptly:

  • Acute hemolysis with hemoglobinuria after cold exposure or viral illness
  • Severe anemia, jaundice, or hemodynamic compromise in children
  • Persistent or recurrent symptoms despite avoidance of cold
  • Adult patients with suspected syphilis or hematologic malignancy
  • Need for confirmatory Donath-Landsteiner testing or hematology consultation
