What is Paget's Bone Disease - Detailed?

Paget's disease is a focal bone disease seen in 1-2% of the population over 55; the male:female ratio is 3:2 and it is more common in the United Kingdom, Australia, the United States and Western Europe.

It typically progresses through three phases: first, increased osteoclast activity (lytic phase); second, exaggerated osteoblast response (mixed phase); third, slow burnout (sclerotic phase). It most commonly involves the pelvis, spine, skull, femur and tibia.

Classic complications include 'sabre tibia,' enlarged skull, hearing loss, secondary osteoarthritis, pathologic fracture, high-output heart failure and rarely osteosarcoma. Treatment is intravenous bisphosphonate (zoledronate) and pain management.

Symptoms

Bone pain (especially at night)
Bone deformity (sabre tibia, skull enlargement)
Hearing loss (skull involvement)
Headache
Joint pain (secondary osteoarthritis)
Pathologic fracture
Often asymptomatic (incidental finding)

Paramyxovirus infection (hypothesis)
