What is Optic Neuritis as First Presentation of MS?

Optic neuritis is acute demyelinating inflammation of the optic nerve causing rapidly progressive vision loss in one eye over hours to days, with pain on eye movement, color desaturation, and a relative afferent pupillary defect.

MRI of the brain and orbits with contrast demonstrates optic nerve enhancement and quantifies central nervous system lesion burden — multiple T2 lesions strongly increase the risk of conversion to multiple sclerosis.

Acute treatment is high-dose intravenous methylprednisolone, which speeds visual recovery; long-term prognosis depends on timely initiation of disease-modifying therapy in patients with high MS conversion risk.
