What is Nutritional Support in Hematology?

Nutritional support in hematology refers to the systematic assessment and management of dietary intake, body composition, and metabolic status in patients with hematologic malignancies, bone marrow failure, and those undergoing hematopoietic stem cell transplantation. It is now considered an integral part of multidisciplinary cancer care.

Patients are at heightened risk of weight loss, sarcopenia, and protein-energy malnutrition due to anorexia, nausea, taste changes, mucositis, graft-versus-host disease of the gut, and prolonged neutropenia. Each of these can independently worsen outcomes and reduce tolerance to curative therapy.

Modern protocols use validated screening tools, indirect calorimetry where available, and stepwise interventions ranging from dietary counseling and oral nutritional supplements to enteral tube feeding and parenteral nutrition for severe gut dysfunction.

Symptoms

Unintentional weight loss greater than five percent in three months
Reduced oral intake from mucositis or nausea
Loss of muscle mass and grip strength
Fatigue and reduced physical performance
Edema or low serum albumin in advanced cases

Risk Factors

Acute leukemia and high-dose induction chemotherapy
Allogeneic stem cell transplantation
Gastrointestinal graft-versus-host disease
Severe mucositis and prolonged neutropenia
Older age, baseline frailty, and prior weight loss
