What is Norovirus Gastroenteritis?

Norovirus is the most common viral cause of acute gastroenteritis worldwide. It causes an estimated 685 million cases and 200,000 deaths annually. Transmission occurs by the fecal-oral route, contaminated food and water, droplets, and fomites; a very low infectious dose is sufficient (18 viral particles).

The incubation period is 12-48 hours. Acute, self-limited illness presents with nausea, vomiting, diarrhea, abdominal pain, and fever. It usually resolves within 24-72 hours, but elderly patients, children, and immunocompromised hosts may experience serious dehydration-related complications.

In closed-environment outbreaks, rapid diagnosis (RT-PCR) and infection control measures are critical. Patients remain contagious for 48-72 hours after symptoms resolve.

Symptoms

Sudden-onset nausea and vomiting
Watery diarrhea (non-bloody)
Abdominal cramps
Low-grade fever
Headache and myalgia
Dehydration (especially in the elderly)

Risk Factors

Living in closed environments (nursing homes, dormitories, ships)
Advanced age
Immunosuppression
Healthcare workers
Consumption of contaminated food

When to See a Doctor?

If you experience any of the following symptoms, seek medical attention promptly:

  • If signs of dehydration develop (dry mouth, oliguria, dizziness)
  • If gastroenteritis occurs in elderly or immunosuppressed patients
  • If bloody diarrhea or high fever accompanies illness (rule out bacterial cause)
  • If improvement does not occur within 48-72 hours

Treatment Methods

Oral rehydration (the cornerstone — ORS solutions)

IV fluid replacement (if oral intake not tolerated)

Antiemetic (ondansetron — for severe vomiting)

Antibiotics are NOT INDICATED (viral etiology)

Infection control: hand hygiene, surface disinfection (chlorine-based), isolation

Continue isolation for 48 hours after symptoms resolve
