Nocturia and Nocturnal Polyuria

Need to wake one or more times per night to void, often associated with disproportionate urine production overnight (nocturnal polyuria), with multifactorial causes ranging from bladder dysfunction to systemic and circadian disorders.

What is Nocturia and Nocturnal Polyuria?

Nocturia is the complaint of waking from sleep at night to void, with each void being preceded and followed by sleep, increasing in prevalence with age in both sexes.

Nocturnal polyuria is defined as overnight urine output greater than 33 percent of total 24-hour urine volume in older adults or greater than 20 percent in younger adults.

Causes are categorized into bladder storage problems (overactive bladder, low capacity), nocturnal polyuria (cardiac failure, sleep apnea, renal concentrating defects), global polyuria (diabetes), and sleep disorders.

Symptoms

Waking one or more times per night to void with disturbed sleep
Daytime fatigue and reduced quality of life from sleep fragmentation
Increased fall risk in older adults walking to the bathroom
Associated symptoms — leg edema, orthopnea, snoring, dry mouth
Polyuria during the day in patients with diabetes mellitus or insipidus

Risk Factors

Older age and male sex with bladder outlet obstruction
Heart failure or peripheral edema with redistribution of fluid at night
Obstructive sleep apnea with increased atrial natriuretic peptide release
Late-evening fluid intake or alcohol and caffeine use
Use of evening diuretics or selective serotonin reuptake inhibitors

When to See a Doctor?

If you experience any of the following symptoms, seek medical attention promptly:

  • Persistent nocturia of two or more episodes per night affecting sleep
  • Daytime sleepiness, falls, or reduced quality of life
  • Coexisting heart failure, diabetes, or sleep-disordered breathing
  • Suspected nocturnal polyuria documented on a 3-day frequency-volume chart
