What is Neurogenic Bladder?

Neurogenic bladder is the general term for the dysfunctions arising from damage to the nerve pathways that regulate bladder storage and voiding functions. It can present as overactive (hyperreflexic) or underactive (areflexic).

Spinal cord injury, multiple sclerosis, Parkinson's disease, diabetic neuropathy, and spina bifida are the main causes. The disorder seriously affects the patient's quality of life and, if not managed, can also lead to renal damage.

Treatment of neurogenic bladder has two main goals: protecting the kidneys (achieving low bladder pressure) and supporting social continence. To achieve this goal, an individualized treatment plan guided by urodynamics is essential.

Symptoms

Urinary incontinence or inability to urinate
Inability to fully empty the bladder (increased residual urine)
Frequent urination or inability to urinate at all
Recurrent urinary tract infections
Autonomic dysreflexia in spinal cord injury (hypertension, sweating — emergency)
Signs of renal failure (in the long term if not managed)

Risk Factors

Spinal cord injury or diseases
Multiple sclerosis and Parkinson's disease
Diabetic neuropathy
Pelvic surgery (nerve damage)
Spina bifida and other congenital neural tube defects
Stroke and brain injury

When to See a Doctor?

If you experience any of the following symptoms, seek medical attention promptly:

  • Urology evaluation after a neurological diagnosis (spinal cord patient, MS, etc.)
  • Bladder emptying assessment in the presence of recurrent UTI
  • Complaints of urinary incontinence or inability to urinate
  • Detection of hydronephrosis on renal ultrasonography
  • Episodes of autonomic dysreflexia (emergency)
