What is Neonatal Group B Streptococcal Infection?

Group B Streptococcus (Streptococcus agalactiae) colonizes the genital and gastrointestinal tracts and may be transmitted to the newborn during delivery. Early-onset disease occurs within the first week of life, while late-onset disease appears between 7 and 90 days.

Sepsis, meningitis, and pneumonia are the principal clinical syndromes, with high mortality and risk of long-term neurologic sequelae if untreated.

GBS screening during pregnancy (35-37 weeks) and intrapartum antibiotic prophylaxis (penicillin or ampicillin) reduce the incidence of early-onset disease.

Symptoms

Respiratory distress in the newborn
Irritability and feeding difficulty
Fever or hypothermia
Signs of meningitis (bulging fontanelle)
Signs of pneumonia

Risk Factors

Maternal GBS colonization
Preterm delivery
Prolonged rupture of membranes
Intrapartum fever
History of a previous infant with GBS disease

  • If fever or hypothermia develops
  • If a seizure or high-pitched cry is observed
  • If a high-risk infant clinically deteriorates
