What is Necrotizing Fasciitis?

Necrotizing fasciitis is an infection that rapidly destroys subcutaneous soft tissues and fascia, usually polymicrobial (Type I) or monomicrobial (Type II, especially Group A streptococci).

Early findings are disproportionately severe pain following a small ulcer or trauma. Pain disproportionate to the clinical appearance and systemic toxicity are characteristic.

It is a dermatologic and surgical emergency requiring urgent surgical debridement and broad-spectrum antibiotic treatment.

Symptoms

Severe pain disproportionate to the clinical findings
Rapidly progressing redness and swelling
Fever, chills, and systemic toxicity
Purple-gray discoloration and bullae on the skin
Crepitus (in gas-producing organisms)
Confusion and hypotension
Progressive necrosis of subcutaneous tissue

Risk Factors

Diabetes
Obesity
Peripheral vascular disease
Immunosuppression
Trauma, surgical wounds, or insect bites
Intravenous drug use
Alcoholism and liver disease

When to See a Doctor?

If you experience any of the following symptoms, seek medical attention promptly:

  • When severe pain and rapidly spreading redness occur on the skin (emergency)
  • When high fever and altered consciousness develop after a small wound
  • When purple spots and bullae appear on the skin
  • When 'crackling' sensation is detected under the skin
  • If the general condition is rapidly deteriorating
