What is Myxedema Coma?

Myxedema coma is the decompensated state of long-untreated or uncontrolled severe hypothyroidism, characterized by hypothermia, bradycardia, hypoventilation, hyponatremia and altered consciousness, and is a medical emergency.

It is more common in elderly women during winter. Infection, sedative drugs, surgery and cold exposure may serve as triggers. Co-existing hypocortisolemia requires additional treatment.

Treatment includes intravenous levothyroxine, liothyronine if needed, glucocorticoids, rewarming, ventilatory support and treatment of infection. Mortality is high so early intervention is critical.

Symptoms

Hypothermia and cold skin
Bradycardia and hypotension
Hypoventilation and hypercapnia
Altered consciousness, stupor, coma
Hyponatremia
Periorbital edema and myxedematous skin
Heart failure

Risk Factors

Long-untreated hypothyroidism
Elderly female sex
Winter months
Infection and sepsis
Use of sedatives
Surgery and trauma
Concomitant hypoadrenalism

  • Hypothermia plus bradycardia plus altered consciousness
  • Acute deterioration in chronic hypothyroidism
  • Hypoventilation and hypercapnia
  • Hyponatremia plus myxedematous appearance
  • Elderly patient with infection plus loss of consciousness
  • History of thyroid surgery plus acute presentation
