What is Multiparametric Prostate MRI (mpMRI)?

Multiparametric prostate MRI (mpMRI) is an advanced imaging method that combines anatomical (T2-weighted) and functional (diffusion-weighted and dynamic contrast-enhanced perfusion) sequences.

PI-RADS v2.1 scoring grades the likelihood that a detected lesion represents clinically significant prostate cancer from 1 to 5. Biopsy is recommended for lesions with a score of 3 or higher.

Owing to its high negative predictive value, it helps avoid unnecessary biopsies, enables targeted sampling with MR-TRUS fusion biopsy, and contributes to treatment planning.

Symptoms

High or rising PSA
Suspicion of cancer despite a negative biopsy history
Need for staging
Active surveillance planning
Mapping before focal therapy
Suspected recurrence after radiotherapy
Lesion detection before biopsy

Risk Factors

Metallic implants (MRI-incompatible)
Claustrophobia
Renal failure (contrast agent)
Pacemaker (incompatible)
Active rectal pathology
Movement disorder
History of pelvic surgery

When to See a Doctor?

If you experience any of the following symptoms, seek medical attention promptly:

  • Before biopsy decision in high PSA
  • Rising PSA despite previous negative biopsy
  • Follow-up during active surveillance
  • Surgery/radiotherapy planning
  • Suspected recurrence

Treatment Methods

01
mpMRI acquisition with standard protocol
02
PI-RADS scoring
03
Lesion mapping for targeted biopsy
04
Guidance of MR-TRUS fusion biopsy
05
Integration with radiotherapy/radical surgery for treatment planning
06
Use as a key tool in active-surveillance decisions
