What is Mini-PCNL (Miniaturized Percutaneous Nephrolithotomy)?

Mini-PCNL is a miniaturized version of standard percutaneous nephrolithotomy using sheath sizes ranging from 12 to 22 French to access the renal collecting system through a smaller percutaneous tract for kidney stone removal.

Compared to standard PCNL (24-30 Fr), mini-PCNL reduces bleeding, transfusion needs, and tissue trauma while maintaining stone-free rates for moderate-sized stones (1-2 cm). It bridges the gap between flexible ureteroscopy and conventional PCNL.

Variants include super-mini-PCNL (10-14 Fr) and ultra-mini-PCNL (6-8 Fr) for even smaller stones. Vacuum-cleaning suction sheaths allow improved stone evacuation and lower intrarenal pressure, reducing infection risk.

Symptoms

Flank pain from kidney stones
Hematuria
Recurrent urinary tract infections
Stones 1-2 cm not amenable to ureteroscopy
Failed shock wave lithotripsy
Lower pole stones with poor anatomy
Stones in patients on anticoagulants
