What is Middle Ear Prosthesis Surgery?

Ossiculoplasty refers to surgical reconstruction of the auditory ossicles damaged by chronic otitis media, cholesteatoma, or trauma. Partial (PORP) and total (TORP) ossicular replacement prostheses are used; biocompatible materials include titanium, hydroxyapatite, and autologous incus.

In otosclerosis, stapedectomy/stapedotomy is performed; the stapes footplate is opened with a laser and a titanium or Teflon piston is placed between the incus and oval window. Laser stapedotomy is an atraumatic technique with high closure rates of the air-bone gap.

Success depends on patient selection, surgical experience, and middle ear anatomy. Outcomes have improved markedly with refined microsurgical and laser techniques, with hearing closure rates frequently exceeding 70-80% in experienced centers.
