What is Differential Diagnosis of Metabolic Alkalosis?

Metabolic alkalosis is a primary acid-base disorder of elevated arterial pH and serum bicarbonate, often accompanied by compensatory hypoventilation.

Chloride-responsive forms include vomiting, nasogastric drainage, diuretic use and chloride-losing diarrhoea; urinary chloride is below 25 mmol/L.

Chloride-resistant forms with urinary chloride above 40 mmol/L are seen in primary hyperaldosteronism, Cushing syndrome, severe hypokalaemia and Bartter or Gitelman syndromes.
