What is Managing Radiotherapy Side Effects?

Radiotherapy uses high-energy ionising radiation to damage the DNA of cancer cells and destroy them. The radiation can affect both the tumour and nearby healthy tissue, which shapes the side-effect profile. Advanced techniques such as IMRT, VMAT and proton therapy have reduced the dose reaching healthy tissue substantially.

Radiation side effects fall into two main groups: acute side effects, which appear during or shortly after treatment and are usually transient, and late side effects, which develop months or years later and may be permanent. The type and severity depend on the area irradiated and the dose received.

Giving the patient comprehensive information about expected side effects before radiotherapy and planning multidisciplinary follow-up during treatment has a direct effect on quality of life. Supportive-care options continue to expand.

Symptoms

Skin reactions: redness, peeling and ulceration in the irradiated area
Fatigue
Mucositis: mouth sores and swallowing difficulty with head-and-neck radiotherapy
Diarrhoea with pelvic or abdominal radiotherapy
Bladder irritation with pelvic radiotherapy — frequent and painful urination
Pneumonitis with lung radiotherapy — cough and shortness of breath (acute)
Late effects: fibrosis, lymphoedema, second cancer and hormonal suppression

Risk Factors

High radiation dose and large treatment volume
Proximity of nearby critical organs
Concurrent chemotherapy (radiosensitisation)
Smoking (increases skin reactions)
Diabetes and connective-tissue disease (for late effects)

When to See a Doctor?

If you experience any of the following symptoms, seek medical attention promptly:

  • Contact the radiation oncologist when skin reactions worsen during treatment
  • Seek urgent assessment for pneumonitis — dry cough and shortness of breath after radiotherapy
  • Arrange dietitian consultation for nutritional support if dysphagia worsens
  • Take part in long-term surveillance programmes for late effects (fibrosis, lymphoedema)
