What is Managing Chemotherapy Side Effects?

Chemotherapy covers drugs that target rapidly dividing cancer cells; the same mechanism can also affect some healthy tissue, which gives rise to the characteristic side-effect profile. The type and severity of side effects vary considerably with the drug, the dose, the route of administration and the patient's individual features.

Supportive care (antiemetics, growth factors, mucositis prophylaxis and so on) is now an integral part of chemotherapy, and a large proportion of side effects can be prevented or kept under control. It is very important to give the patient thorough information before starting chemotherapy and to plan follow-up carefully.

During treatment, oncology nurses and the supportive-care team play a critical role in the early detection and management of side effects. The patient should keep an open line of communication with the oncology team for any problem that arises during treatment.

Symptoms

Nausea and vomiting
Hair loss (alopecia)
Fatigue and weakness
Mouth sores (mucositis)
Neutropenia: increased risk of infection and fever
Thrombocytopenia: increased tendency to bleed
Peripheral neuropathy: numbness and tingling in the hands and feet

Risk Factors

High-dose chemotherapy regimens
Combination chemotherapy protocols
Advanced age and poor performance status
Pre-existing kidney, liver or cardiac disease
Genetic differences in drug metabolism (pharmacogenetics)

When to See a Doctor?

If you experience any of the following symptoms, seek medical attention promptly:

  • When fever exceeds 38 degrees Celsius after chemotherapy — treat as neutropenic fever and seek emergency care
  • With uncontrolled nausea and vomiting and an inability to drink enough fluids
  • With severe numbness or weakness in the hands and feet
  • With severe skin or mucosal reactions or ulceration
  • With severe fatigue, shortness of breath or chest pain
