What is Male Osteoporosis (Endocrine)?

Male osteoporosis is a metabolic bone disease characterized by reduced bone mineral density and increased fracture risk in men. It affects approximately 1 in 5 men over 50 and is significantly underdiagnosed compared to women.

Up to 50-60% of cases are secondary to identifiable causes, particularly endocrine disorders. Hypogonadism (primary, secondary, or androgen deprivation therapy), hyperparathyroidism, hyperthyroidism, glucocorticoid excess and growth hormone deficiency are major endocrine etiologies.

Diagnosis requires DEXA scan with T-score ≤-2.5 or fragility fracture history, alongside thorough endocrine workup. Treatment combines lifestyle measures, calcium-vitamin D, hormone replacement when indicated, and antiresorptive or anabolic agents.

Symptoms

Fragility fractures (vertebra, hip, wrist)
Loss of height
Back pain (often from vertebral fractures)
Kyphosis
Reduced grip strength
Symptoms of hypogonadism (low libido, fatigue)
Symptoms of underlying endocrine disease

Risk Factors

Hypogonadism (primary or secondary)
Long-term glucocorticoid therapy
Hyperparathyroidism
Hyperthyroidism
Androgen deprivation for prostate cancer
Smoking and alcohol abuse
Family history and low body weight
