What is Macroprolactinoma?

Macroprolactinoma is a prolactin-secreting pituitary adenoma 10 mm or larger in size. Microprolactinomas typically occur in women, while macroprolactinomas are more common in men. Because symptoms develop insidiously in men, the diagnosis is often made when the tumor has already grown.

High prolactin levels suppress gonadotropin secretion, leading to amenorrhea-galactorrhea in women and decreased libido and impotence in men. As the tumor enlarges, headache, visual field defects (bitemporal hemianopia), and hypopituitarism may develop.

Dopamine agonists are the first-line treatment; surgery and radiotherapy are usually reserved for patients who are resistant to or intolerant of medical therapy.

  • Unexplained galactorrhea with headache
  • Narrowing of the visual field
  • Sexual dysfunction and infertility
  • When elevated prolactin is detected
  • Tumor growth or signs of apoplexy under therapy (emergency)
