Lymphocytic Colitis

What is Lymphocytic Colitis?

Lymphocytic colitis is one of the two main types of microscopic colitis; female predominance is less marked than in collagenous colitis (female:male 1.5-3:1) and the mean age is 50-60.

Drugs (NSAIDs, lansoprazole, sertraline, ranitidine), autoimmune diseases, smoking, viral infections, and bile acid malabsorption are implicated; histologically, an increased number of intraepithelial lymphocytes (>20/100 enterocytes) is the hallmark.

Diagnosis is made by colonoscopic random biopsy. Budesonide 9 mg/day produces remission in 80%, but recurrence rate is 50-70%. Smoking cessation, drug review, and lifestyle changes are important.

Symptoms

Chronic non-bloody watery diarrhea
Nocturnal diarrhea (50%)
Cramping abdominal pain
Bloating
Mild weight loss
Fatigue
Fecal urgency

Risk Factors

Age 50-60
Autoimmune diseases (celiac most common)
Drugs (NSAIDs, PPIs, SSRIs)
Smoking (active or former)
Family history
HLA-DQ2 positivity
Mostly female sex
