What is Lymph Node Swelling?

Lymph node swelling is enlargement of lymph nodes (>1 cm in adults; >1.5 cm in inguinal region). It may be reactive, infectious or neoplastic in origin.

Cervical, axillary, supraclavicular and inguinal regions are commonly involved. Supraclavicular adenopathy strongly suggests malignancy. Generalized lymphadenopathy is associated with HIV, EBV, lymphoma and autoimmune disease.

Workup is guided by clinical features and may include complete blood count, serology, ultrasound, fine-needle aspiration and excisional biopsy.

Symptoms

Palpable mass in cervical, axillary or inguinal region
Tender or non-tender nodes
Fever and night sweats
Weight loss
Fatigue and malaise
Local skin redness
Sore throat or upper respiratory infection
Difficulty swallowing

Risk Factors

Recent upper respiratory infection
Tuberculosis exposure
HIV, EBV, CMV infection
Autoimmune diseases (SLE, RA)
Family history of lymphoma
Tobacco and alcohol use
Tattoos and chronic skin trauma
Immunosuppression

When to See a Doctor?

If you experience any of the following symptoms, seek medical attention promptly:

  • Hard, fixed nodes greater than 2 cm
  • Persistence beyond 4 weeks
  • Supraclavicular lymphadenopathy
  • B-symptoms (fever, sweating, weight loss)
  • Generalized lymphadenopathy
  • Rapidly enlarging node
