What is Long-Term Cancer Survivorship?

Cancer survivorship encompasses the period from cancer diagnosis through the remainder of life, with the long-term phase typically defined as five or more years after primary treatment completion.

Comprehensive survivorship care addresses surveillance for recurrence, monitoring of treatment-related late effects (cardiotoxicity, neurotoxicity, endocrine, fertility, second malignancies), psychosocial well-being, and lifestyle modifications.

Treatment summaries and survivorship care plans (SCPs) developed by Institute of Medicine recommendations facilitate transition from oncology to primary care.

Symptoms

Cancer-related fatigue persisting beyond active treatment
Chemotherapy-induced peripheral neuropathy with chronic numbness/pain
Cardiotoxicity from anthracyclines or HER2-targeted therapy: heart failure, cardiomyopathy
Cognitive impairment ('chemo brain') with memory and concentration deficits
Lymphedema after axillary or pelvic node dissection
Endocrine sequelae: hypothyroidism, hypogonadism, premature menopause
Psychosocial: anxiety, depression, fear of recurrence, post-traumatic stress
Sexual dysfunction and altered body image

Risk Factors

Anthracycline-based chemotherapy (cumulative dose dependent cardiotoxicity)
Chest or mediastinal radiation (breast cancer, lymphoma) — secondary cancers, cardiac disease
Pelvic radiation — secondary cancers, infertility, bowel/bladder dysfunction
Allogeneic stem cell transplantation — graft-versus-host disease, secondary malignancies
Immunotherapy — chronic immune-related adverse events
Pediatric or young adult cancer — longer follow-up window, growth/development impact
Hereditary cancer syndromes — second cancer risk

When to See a Doctor?

If you experience any of the following symptoms, seek medical attention promptly:

  • New persistent symptoms after treatment completion (recurrence concern)
  • Cardiac symptoms in patient with prior anthracycline or chest radiation
  • Worsening neuropathy or new neurologic deficit
  • Persistent fatigue, sleep disturbance, or mood changes
  • Sexual dysfunction, infertility concerns, or hormonal symptoms
  • Routine surveillance visits per cancer-specific guidelines (e.g., annual mammogram, colonoscopy)
