What is Laparoscopic Vesicovaginal Fistula Repair?

Vesicovaginal fistula is an abnormal communication between the bladder and vagina that produces continuous urinary incontinence; the most common etiologies in developed countries are gynaecologic surgery, while obstetric injury predominates in developing regions.

Surgical repair is the gold standard. The laparoscopic and robotic transabdominal approach is preferred for high, recurrent or large fistulae and provides excellent visualization and tension-free closure with tissue interposition.

Outcome depends on early diagnosis, adequate preoperative imaging, mature non-inflamed tissue, multilayered closure with interposition flap and prolonged urinary diversion.

Symptoms

Continuous urinary leakage from vagina
Recurrent urinary tract infections
Vulvovaginal irritation
Pelvic discomfort
Hematuria after gynaecologic surgery
Postpartum incontinence after obstructed labour
Failed conservative catheter management

Risk Factors

Hysterectomy or pelvic surgery
Obstructed labour
Pelvic radiation
Pelvic malignancy
Foreign body or pessary erosion
Endometriosis surgery
Inflammatory bowel disease
