Laparoscopic Right Hemicolectomy

Minimally invasive resection of the right colon for cancer, Crohn's, or benign disease with intracorporeal or extracorporeal anastomosis.

What is Laparoscopic Right Hemicolectomy?

The operation removes the right colon with en-bloc ligation of the ileocolic, right colic, and right branch of the middle colic vessels.

Lymphadenectomy follows ASCRS and JSCCR principles — D2 or D3/CME for cancer, harvesting at least 12 nodes.

Reconstruction uses an ileocolic anastomosis (stapled side-to-side or hand-sewn end-to-side) — intracorporeal anastomosis has gained popularity with the COLOR trial era.

The laparoscopic approach shortens ileus, reduces wound complications, and provides oncologic outcomes equivalent to open surgery per COST, CLASICC, and COLOR trials.

Symptoms

Indications include:
Right-sided colon adenocarcinoma (cecum, ascending, hepatic flexure)
Complicated ileocolic Crohn's disease with stricture or fistula
Large right-sided adenomas not amenable to endoscopic resection
Appendiceal neoplasms (mucinous, carcinoid >2 cm, goblet cell)
Cecal volvulus or ischemic right colon

Risk Factors

Anastomotic leak (2–4%) with potential for sepsis and reoperation
Postoperative ileus, especially with extensive mobilization
Bleeding from the ileocolic pedicle or gastroepiploic arcade
Duodenal or ureteral injury during Kocher and retroperitoneal dissection
Conversion to open surgery in adhesive or obese cases
Incisional hernia at extraction site

When to See a Doctor?

If you experience any of the following symptoms, seek medical attention promptly:

  • New-onset iron-deficiency anemia, hematochezia, or change in bowel habit warrants colonoscopy
  • Crohn's disease with obstructive symptoms, abscess, or persistent fistula
  • Surveillance colonoscopy identifying a lesion not amenable to endoscopic removal
