What is Laparoscopic Radical Prostatectomy with Lymphadenectomy?

Laparoscopic radical prostatectomy is a surgical procedure designed for prostate cancer confined to the prostate. The prostate, seminal vesicles and pelvic lymph nodes are excised through laparoscopic access via 4-5 trocars, with vesicourethral anastomosis at the end.

Extended pelvic lymphadenectomy includes obturator, internal iliac, external iliac and presacral lymph nodes. It is recommended when nomogram-predicted lymph-node involvement risk is >5%, particularly for intermediate- and high-risk patients.

Compared with open surgery, this procedure offers reduced blood loss, less postoperative pain, shorter hospital stay and earlier return to normal activity. Long-term oncologic and functional outcomes (continence, potency) are equivalent.

Bleeding (vascular plexus)
Urinary incontinence (10-20%)
Erectile dysfunction (50-70%)
Vesicourethral anastomosis stricture
Lymphocele formation
Rectal injury (rare, <1%)
