What is Lacrimal Gland Degeneration?

The lacrimal gland produces the aqueous component of tears that lubricates and nourishes the ocular surface

Degeneration involves acinar atrophy, fibrosis, and loss of secretory cells over time

The condition is a leading cause of aqueous-deficient dry eye disease, particularly in older adults

Histopathology reveals lymphocytic infiltration, ductal dilation, and parenchymal replacement by fat

Both age-related and autoimmune (Sjögren) forms exist with overlapping features

Symptoms

Persistent dryness, grittiness, and foreign body sensation in the eyes
Burning, stinging, and photophobia worsening throughout the day
Blurred or fluctuating vision relieved by blinking
Mucous discharge and tired heavy eyelids
Reflex tearing in cold or windy conditions despite chronic dryness

Risk Factors

Advanced age (over 60 years) with natural lacrimal involution
Female sex with postmenopausal hormonal changes
Sjögren syndrome and other systemic autoimmune diseases
Long-term contact lens wear and refractive surgery history
Medications such as antihistamines, antidepressants, and beta-blockers
