What is Japanese Encephalitis?

Japanese encephalitis virus (JEV) is a virus from the Flavivirus family. The natural cycle is maintained between mosquitoes-pigs/birds; humans are intermediate hosts.

Tens of thousands of cases are reported annually in Asia and the Western Pacific. Most cases are asymptomatic; a significant portion of symptomatic cases present with severe encephalitis.

Mortality reaches 20-30%, sequela rate 30-50%. Vaccine is the most effective protection method.

Symptoms

Fever, headache, vomiting
Consciousness changes
Seizures (especially in children)
Neck stiffness and meningismus
Parkinsonism-like movement disorders
Muscle weakness or paralysis
Psychiatric changes

Risk Factors

Being in rural areas in Asia and Western Pacific
Areas with rice paddies and animal husbandry
Unvaccinated travel
Being outdoors during monsoon and rainy seasons
Pediatric and young age
Old age
Immunosuppression

When to See a Doctor?

If you experience any of the following symptoms, seek medical attention promptly:

  • If fever and headache occur after travel to endemic area
  • When seizures, confusion, or neck stiffness develop
  • When sudden fever and behavioral change occur in a child
  • When muscle weakness and partial paralysis emerge
  • In case of suspected exposure in unvaccinated individuals

Treatment Methods

01
Supportive intensive care
02
Seizure management and intracranial pressure control
03
Maintenance of fluid-electrolyte balance
04
Mechanical ventilation when respiratory support is needed
05
Rehabilitation (if sequelae are present)
06
Protection with Japanese encephalitis vaccine
