Ischemic Colitis

What is Ischemic Colitis?

Ischemic colitis is the exposure of the bowel wall to ischemia as a result of a temporary or permanent decrease in colon perfusion. The splenic flexure and rectosigmoid region (watershed areas) are most commonly affected.

Low-flow conditions (hypotension, heart failure), atherosclerotic vascular disease, vasculitis, and conditions following aortic surgery are the main causes.

Most cases are non-transmural and transient (reversible) and respond to conservative treatment. The gangrenous form is an emergency with high mortality.

Symptoms

Sudden onset of left lower abdominal pain
Bloody or maroon-colored stool
Nausea
Urgency to defecate
Mild fever
Distension
Signs of peritonitis in gangrene

Risk Factors

Advanced age
Atherosclerotic vascular disease
Episodes of hypotension and heart failure
History of aortic surgery
Vasculitis and thrombophilia
Dialysis and shock
Some medications (digital, cocaine, NSAIDs)

If you experience any of the following symptoms, seek medical attention promptly:

  • Sudden abdominal pain and bloody stool in elderly patient
  • Symptoms with uncontrolled diabetes and vascular disease
  • Abdominal pain after aortic surgery
  • Signs of peritonitis
  • Patient unable to tolerate fluids
