What is Ipilimumab plus Nivolumab for Malignant Pleural Mesothelioma?

Ipilimumab plus nivolumab combination provides first-line immunotherapy for malignant pleural mesothelioma.

CheckMate 743 trial demonstrated overall survival benefit versus platinum-pemetrexed chemotherapy.

Dual checkpoint blockade combines anti-CTLA-4 ipilimumab with anti-PD-1 nivolumab.

Benefit most pronounced in non-epithelioid sarcomatoid and biphasic histology.

Treatment continues for two years total with 21-day cycles.

Symptoms

Immune-related adverse events including pneumonitis, colitis, hepatitis, endocrinopathies require monitoring.
Severe pneumonitis is potentially life-threatening requiring imaging and immediate treatment.
Colitis with diarrhea requires steroid therapy and infection workup.
Hepatitis with transaminase elevation requires monitoring with steroid therapy when severe.
Endocrinopathies including thyroid dysfunction, hypophysitis, adrenal insufficiency require hormone replacement.

Risk Factors

Asbestos exposure represents primary etiologic factor in pleural mesothelioma development.
Occupational exposure including shipyard work, construction, mining contributes to mesothelioma risk.
Latency period of 20-40 years between asbestos exposure and mesothelioma diagnosis.
Erionite exposure in certain geographic regions causes mesothelioma without asbestos.
BAP1 germline mutations associate with mesothelioma and other cancer susceptibility.
