What is Intraoperative Ultrasound for Surgical Guidance?

Intraoperative ultrasound (IOUS) is the application of high-frequency sonography during surgery to provide real-time anatomic and pathologic information. It is widely used in hepatobiliary surgery to detect occult liver metastases, define vascular relationships and plan parenchymal-sparing resections.

Beyond hepatobiliary applications, IOUS is essential in pancreatic surgery, neurosurgery for tumor localization, breast surgery for occult lesions, urologic surgery for partial nephrectomy and endocrine surgery for parathyroid and thyroid lesions. Contrast-enhanced and Doppler modalities expand diagnostic capabilities.

IOUS reduces the incidence of incomplete resection, helps avoid injury to vital structures and supports minimally invasive approaches. Operator training and integration with cross-sectional imaging are crucial for optimal results.

Symptoms

Need to localize occult tumors
Suspected vascular invasion
Liver metastases assessment
Pancreatic mass mapping
Parathyroid adenoma localization
Margin assessment intraoperatively
Complex anatomy requiring guidance

Risk Factors

Multiple liver metastases
Hepatobiliary surgery
Recurrent endocrine tumor surgery
Pancreatic neuroendocrine tumors
Reoperative surgery
Lack of preoperative localization
Need for parenchymal-sparing surgery

  • Planning hepatobiliary resection
  • Recurrent or multifocal tumors
  • Need for occult lesion detection
  • Reoperative endocrine surgery
  • Parenchymal preservation desired
  • Complex tumor-vascular relationships
