What is Intestinal Stoma Revision Surgery?

Intestinal stomas (ileostomy, colostomy, jejunostomy) are commonly created during emergency or elective procedures for inflammatory bowel disease, malignancy, diverticulitis, trauma or pelvic surgery. Up to 30% of patients experience stoma-related complications during follow-up, requiring revision surgery.

Common indications for revision include parastomal hernia (most common late complication), prolapse, stenosis, retraction, mucocutaneous separation, peristomal pyoderma gangrenosum and refractory dermatitis. Malposition causing leakage, pouching difficulty or impaired quality of life is also a frequent indication. Stoma site relocation may be needed.

Surgical options range from local stoma reconstruction at the same site to laparotomic relocation, mesh-reinforced parastomal hernia repair (Sugarbaker, keyhole, sandwich techniques), laparoscopic-assisted approaches and stoma reversal where appropriate. Choice depends on hernia size, prior surgeries, body habitus, oncologic status and patient preferences.
