What is Inflatable Penile Prosthesis Implantation?

An inflatable penile prosthesis (IPP) is a three-piece hydraulic device consisting of paired cylinders implanted in the corpora cavernosa, a fluid reservoir placed in the retropubic space and a control pump positioned in the scrotum. The prosthesis allows on-demand erection by transferring saline from the reservoir to the cylinders.

It is indicated for organic erectile dysfunction (post-radical prostatectomy, diabetic, vascular, neurogenic, post-priapism, Peyronie's disease) refractory to PDE-5 inhibitors, intraurethral and intracavernosal therapy or vacuum devices. Two-piece and malleable (semirigid) prostheses are alternatives in selected cases.

Surgical approaches include penoscrotal, infrapubic and subcoronal incisions. Modern devices feature antibiotic and hydrophilic coatings to reduce infection risk, with mechanical durability over 10-15 years. Patient and partner satisfaction rates exceed 90% in expert centers, making IPP one of the most satisfying procedures in urology.
