What is Infection Management in Hematology?

Infection management in hematology covers the prevention, diagnosis, and treatment of infections in patients with hematologic malignancies, bone marrow failure, and recipients of cellular therapies. It is one of the most important determinants of survival in modern hematologic care.

These patients face profound and prolonged immunosuppression from disease itself, intensive cytotoxic regimens, monoclonal antibodies, T-cell-depleting therapies, and graft-versus-host disease. Resulting risks include febrile neutropenia, invasive fungal disease, viral reactivation, and atypical bacterial infections.

Care is delivered through structured pathways that include screening, primary and secondary prophylaxis, environmental controls, prompt empiric broad-spectrum antibiotics for febrile neutropenia, and rapid de-escalation guided by cultures, molecular tests, and imaging.

Symptoms

Fever above 38 degrees during neutropenia
Chills, hypotension, or signs of sepsis
Cough, dyspnea, or new pulmonary infiltrates
Diarrhea, abdominal pain, or perianal infection
Skin lesions or central venous catheter site changes

Risk Factors

Acute leukemia and induction chemotherapy
Allogeneic stem cell transplantation
Graft-versus-host disease and prolonged corticosteroids
Use of fludarabine, anti-CD20, and anti-CD52 agents
Prolonged severe neutropenia and central venous catheters
