What is Immune Deficiency Screening in Hematology?

Immune deficiency screening is a systematic evaluation of immune system components in patients presenting with recurrent unusual or severe infections. Screening includes quantitative immunoglobulin levels lymphocyte subsets neutrophil function and complement assays. The goal is identifying primary congenital or secondary acquired immunodeficiency.

Common indications include more than two pneumonias yearly recurrent sinopulmonary infections opportunistic infections failure to thrive autoimmune cytopenias and family history of immunodeficiency. Initial workup measures complete blood count differential immunoglobulin G A M E and isohemagglutinin titers along with vaccine response.

Specialized testing includes lymphocyte enumeration by flow cytometry mitogen response complement CH50 and AH50 neutrophil oxidative burst by dihydrorhodamine and genetic panels for suspected primary immunodeficiency. Referral to clinical immunology is recommended for confirmed or suspected disorders requiring specific therapy.
