What is Hydatid Cyst (Comprehensive)?

Echinococcus granulosus is a small tapeworm whose adult stage parasitizes dogs and other canids. Eggs shed in feces are ingested by intermediate hosts (sheep, cattle, humans), where larvae penetrate intestinal wall, enter portal circulation, and form fluid-filled cysts in liver (70 percent), lungs (20 percent), and rarely brain, bone, kidney, or heart. Endemic regions include the Mediterranean, Middle East, Central Asia, and South America.

Hepatic cysts may grow asymptomatically for years until reaching 10 cm or causing pain, jaundice from biliary compression, or rupture into peritoneum, biliary tree, or pleura with potentially fatal anaphylaxis. Pulmonary cysts present with cough, hemoptysis, or chest pain. World Health Organization ultrasound classification (CE1 to CE5) guides treatment.

Diagnosis combines imaging (ultrasound, computed tomography, magnetic resonance) with serology (ELISA, immunoblot). Active cysts (CE1, CE2, CE3a) require treatment: surgery (open or laparoscopic), percutaneous puncture-aspiration-injection-reaspiration (PAIR) with hypertonic saline or ethanol, or albendazole alone for small cysts. Inactive cysts (CE4, CE5) are watched. Albendazole 400 mg twice daily perioperatively reduces recurrence.

Symptoms

Right upper quadrant pain or mass
Cough, hemoptysis with pulmonary cyst
Allergic reaction or anaphylaxis (rupture)
Jaundice from biliary obstruction
Eosinophilia and positive serology

Sheep and cattle farming areas
Dog ownership in endemic regions
Mediterranean and Middle East origin
Contaminated vegetable consumption
Children with close dog contact
